Legacy of the White Mage by: Mike MacDee (the link here does not have this) Was cannibalized into a Manhunt & The Arden Daredevils.

Someone had a prinout for sale, it was based on the notes of the unreleased Dwarf Quest Pack that are floating around the net.
I just did a search for it again and found the subtitle: Cult of the Black Hand!

Clicked on the contributor's name and he also has Morkar's Dark Dungeon! Plus a D&D Module.
Another edit: I just found the author on https://www.facebook.com/groups/officialheroquest/. He linked to that lulu site so I guess it's legit. Will have to see if he sells PDFs.

Doesn’t appear to follow the notes too closely as the list of new monsters doesn’t match what was in the design notes at all.
