A: All these spear traps are known by Akaroth. If a players opens a door with a spear trap behind it, while Akaroth is in the same room as that player. The spear trap wont go off because Akaroth has warned them.

There is no mention of which D a monster reappears. I think you should specify either "...reappear at the square marked 'D' nearest to its origin" or '"...reappear at any square marked 'D'." I'd change this to: " When any player in this room searches for treasure, tell the players this is the crypt of Zanrath and Boroush. What about spear traps that are further in the room but not behind the door? Are they also ignored if Akaroth is present? If so, must Akaroth be in the room with the further trap, or does he warn from the adjacent room, as with spear traps behind doors?

Stats are needed here, I think. Spells, too. It is possible for Heroes to run down Fanrax before he escapes. He needs a contingency, such as an escape spell.

This person isn't bothered by the presence of the many undead creatures who are wandering around. The undead aren't even bothering him. This must be a great source of evil to be so respected by the undead. Draped in his cloak, with his face under a hood, this person looks from tomb to tomb and checks the names on the graves. If an evil source is doing his own dirty work, then something bad is bound to happen.
