Make sure it is not too easy so that it really is just a chance and not a giveaway. Hr hr hr.

I am considering making a netherworld dungeon for slain heroes to escape and thereby revive themselves in my campaign and nudging it to the Morcar player. That way there's an opportunity to bring your character back without just giving it away.
Not until AtOH and WoM though. Maybe beating Kellars Keep and Return of the Witchlord could earn characters the title of Eternal Champion and grants them a legendary status that affords an opportunity to revive themselves if slain.

That's definitely the tricky part, making it not so easy. Maybe the idea should be that after any given quest the surviving hero(es) delve after the deceased hero(es). This way the more heroes survive, the greater their odds of reviving a lone straggler, vice 1-2 heroes trying to recover 2-3 heroes from the far-side.
God forbid, 1 hero is trying to revive 3.
