I think to avoid a death trap in the adjacent 3x5 room, which is just a dead end, that secret door should be moved over 4 squares to the left and connect the two...

Is there some compendium of all the scenario/map "mistakes" somewhere, to avoid killing everyone on a map mistake "too often"?

The link I posted above covers all the mistakes in the Phoenix quests and some irregularities in a few other "official" releases. Mainly the errors aren't in the originals, but in our fan re-creations of them.
You can find everything you need there! Now that doesn't cover the "empty chests" but it covers the stuff we're talking about here. Actually I've linked just about everything in that thread, so the empty chests link is there, and some other things. Enjoy!

If you are at a Champion level, just go to the North American version of the board on the Homepage and click Against the Ogre Horde and Wizards of Zargon. And scroll down to the Remastered Verdion by drathe.

The quests are only accessible if you are of Champion Status.
